In registration and assessment, I only collect information that is necessary.
Registration data will include information such as your date of birth, family members and GP.
The assessment information is guided by the issues you bring to the consultation and standard psychiatric assessment requirements.
Your information is obviously sensitive and personal. I keep all information secure (paper and electronic) and respect it as if it was my own. Following our appointments, I will write to your GP and this will be copied to you – the young person or your parent / carer depending on your age. I will not share information about you without discussing this with you first. (Except in very rare circumstances relating to safety and safeguarding.) After our appointments your information will be kept and stored according to legal requirements and then destroyed appropriately.
